Would it be usual for the fact that a mother has died to be indicated on a child's baptism entry?

Hm. I've never seen it, so far as I remember. Nor the father, if he were dead. Which isn't to say that it doesn't happen sometimes.

I seem to recall a baptism where mother is referred to a Widow Smith or similar, and I don’t see why iit shouldn’t happen the other way round. It was all down to the vicar, and believe me, I’ve see some brilliant entries in the course of my research, many didn’t hold back in expressing themselves. :D

Thanks. I’ve also seen ‘widow of ...’ but not the other way round.
