Why would a birth be registered twice on the same day. Baby is on the BDM register with mothers maiden name ie baby is named Mary Smith and again registered on the same day with same birth index number but with dads surname ie baby is named Mary Jones. Parents were married at the time of baby's birth so I don't get why they registered the baby in both the mothers maiden name and the fathers surname too, do they get 2 birth certificates then in the name of Mary Smith and Mary Jones?

Please remember that the GRO Index is not a list of registrations. It exists to help us order certificates.

A single registration may be indexed twice for various reasons. The usual reason is that the parents were not married. The assumption is that the child might take either the father's or mother's surname and that's the way the index was drawn up.

The child's surname didn't start appearing on a birth certificate until fairly recently. The child's surname was inferred from the parents' surname or surnames.

You'll need to get the certificate to find out for sure.

As above - it would suggest they weren't married ...... you need to see what is actually on the certificate to be certain, working from indexes always involves making assumptions which can sometimes be wrong.

You also need to understand how the index is compiled - the new GRO index uses different rules to the existing "FreeBMD" index for example.

Yes as Peter and Anthony say it is usually because the parents were unmarried at the time of registration, and the baby was re registered after the marriage took place. We have such an instance in our Family and both registrations were in the same quarter, but they did have different page numbers.
