It says:

Pregnancy.
Puerperal Convulsions.

Google on the 2nd line gives all sorts of answers for its meaning, but quite what it indicates in today's terms, I wouldn't care to say.

I looked it up and it sounds like it could have been preeclampsia before the birth, especially as the certificate says "pregnancy" - that suggests to me that the baby had not yet been born.

This is likely to be a case of Puerperal sepsis bacterial infection causing septicaemia, which would result in fever & convulsions. It was a common cause of death in or post childbirth before the introduction of proper hygiene practices were introduced. Work by Semmelweis in a Vienna hospital around 1850. He showed deaths on the maternity ward were doctors fresh from post mortem work had much higher mortality than the ward looked after by nurses in clean uniforms and good hand washing protocols. Sadly it took many years before his ideas were understood and put into practice.
